  • Rotator cuff strain or tear— the rotator cuff is a group of four muscles and their tendons that stabilize the shoulder joint. Overuse, sudden overload (like catching a heavy fall), or repetitive overhead activity can strain or tear these structures.
  • Shoulder impingement syndrome— when the rotator cuff tendons or the subacromial bursa are compressed between the bones of the shoulder during arm elevation. Impingement is characterized by pain with reaching overhead or behind the back.
  • Bursitis— inflammation of the subacromial bursa, a fluid-filled sac that cushions the rotator cuff tendons. Often develops alongside impingement or from repetitive motion.
  • AC joint sprain— the acromioclavicular joint at the top of the shoulder can be sprained in falls or contact sports, causing localized pain at the "point" of the shoulder.
  • Frozen shoulder (adhesive capsulitis)— progressive stiffening and pain of the shoulder joint capsule, often following a period of immobilization or without clear cause. Motion becomes severely restricted in all directions.
  • Referred pain from the cervical spine— nerve roots at C5 and C6 supply sensation to the outer shoulder and arm. A bulging disc or misaligned vertebra at these levels can produce shoulder pain that mimics local shoulder pathology.

How Spinal Alignment Affects Shoulder Function

The cervical and thoracic spine have a direct influence on shoulder mechanics that most people don't appreciate. Consider:

  • The nerve roots that control shoulder and arm muscles exit the lower cervical spine. Cervical subluxations at C4–C6 can create weakness, numbness, or altered muscle tone that affects how the shoulder functions.
  • The thoracic spine (mid-back) and the position of the shoulder blades are mechanically linked. When thoracic mobility is reduced — as it commonly is with prolonged sitting and forward posture — the shoulder blade doesn't move properly on the ribcage. This changes how the shoulder joint loads with every arm movement and sets the stage for impingement and rotator cuff problems.
  • A forward head posture, extremely common in today's screen-heavy world, shifts the mechanics of the entire upper body. The shoulder blades round forward, the rotator cuff operates at a mechanical disadvantage, and the risk of impingement increases significantly.
